Key Responsibilities

• Deliver person centred care that upholds resident’s dignity, respects individual choices and promotes independence.
• Use therapeutic communication, active listening and reassurance to build trust and support emotional wellbeing.
• Help prevent pressure damage through appropriate repositioning, mobility support and early identification and reporting of skin changes, in line with individual care plans.
• Recognise changes in resident’s physical health, behaviour or emotional presentation, identifying potential unmet needs and escalating concerns promptly.
• Adapt communication to support residents living with dementia, confusion or distress, using patience, reassurance and gentle redirection.
• Encourage residents to maintain their abilities by participating in daily routines and completing tasks as independently as possible.
• Build meaningful connections through conversation, reminiscence and activities reflecting each resident’s interests, life experiences and abilities.
• Support safe movement, positioning and transfers in accordance with individual care plans, moving and handling assessments and training.
• Promote nutrition and hydration while monitoring changes in appetite, fluid intake, swallowing or general comfort.
• Observe for signs of pain, discomfort or deterioration, ensuring relevant concerns are communicated clearly and recorded accurately.
• Work collaboratively with senior carers, nurses and colleagues, following safeguarding procedures, maintaining confidentiality and reporting concerns to protect residents from harm.

The Successful Candidates

• Proven experience across UK care homes, nursing homes, hospitals or wider adult social care settings.
• Experience supporting individuals with a range of needs, including dementia, complex care, mobility, personal care and maintaining independence.
• A caring, dependable and professional approach, with excellent communication skills and the ability to build positive relationships with service users and colleagues.
• Strong observational skills and sound judgement, with the confidence to recognise changes in wellbeing and report concerns appropriately.
• A good understanding of person centred care, safeguarding, infection prevention and control, and accurate care documentation.
• Current PVG or willingness to apply.
