Are you an experienced Occupational Therapist looking for a challenging new opportunity?

Our client is looking for an Occupational Therapist to work within a busy Re-ablement and Intermediate Care Service. This post is required to provide maternity cover for 26 weeks and to offer operational resilience to promote successful discharges from Acute and Intermediate care services.

Key Tasks include:
Ø Work and communicate as part of a team
Ø Contribute to core function of Re-ablement and Intermediate Care service
Ø Undertake assessments and interventions as required to meet the service needs of the Health and Social Care Partnership and the individuals it serves
Ø Participate in development of care plans and risk assessments
Ø Complete system-based paperwork using SWIFT
Ø Any other tasks and duties commensurate with the grade – as directed by the Team Leader

Essentials Requirements:
· A degree in Occupational Therapy
· Must be registered with the HCPC
· Have demonstrable experience in Re-ablement practices and processes
· Awareness of and current/recent training in: Adult Protection – Practice and Process
· Have knowledge and proficiency to support discharge from hospital/ Intermediate Care
· Hold full driving licence
· Knowledge and experience of SWIFT – desirable
