Sometimes patients remain in hospital for longer than required when their physical, social and emotional needs could be much better met in the home or community. Working with Occupational Therapists, Social Workers, Physiotherapist and other health and social care professionals, our East Renfrewshire based client is seeking a diligent Reablement Discharge Support Worker for a minimum of 6 months to facilitate safe and swift hospital discharge. In doing so the successful Reablement Support Worker with individuals to develop personalised care and support plan and identify where community services such as Care at Home and the wider reablement team may support the promotion and optimisation of independence.

As a Reablement Support Worker in East Renfrewshire, you will be tasked with arranging meeting with individuals to organise their discharge from hospital. You will make preparations for this in accordance with consultations with your health care professional colleagues and having taken into account viewpoints and assistance from family members or carers to ensure they remain involved.

In order to be considered for the role of Reablement Support Worker in East Renfrewshire, you must have:

  • Minimum of SVQ level 3 or HNC in Health and Social Care
  • Experience of working with older people with physical disabilities in a multi-disciplinary team
  • Be a member of the PVG scheme and have the willingness to undergo an update
  • A UK driver’s licence and have access to a care
  • Available to work full-time and for occasional weekend work
